I found that Outlook almost always, if not always, attempts to authenticate using the wrong protocol first, then successfully connects. Predominantly POP users, but some IMAP too.

when we last upgraded, a long time ago, we got bit by Settings->Network->Blacklisted IPs->Temporarily Blocked IP Addresses.

Outlook has a tendency to fail a first login attempt if you put an email address in the username field.  This generates a failed login from an IP.  If an office uses Outlook, there are probably many failed logins per minute, immediately surpassing the default threshold on the "Temporarily Blocked IP Addresses" setting.
